[A] Scope of Service:
1) Contractor needs to provide data entry of forms and documents..
2) Volume FORM I
- Estimated total new applications annually - 20,000.
3) Volume FORM II
- Estimated total forms annually - 830,000.
- Estimated forms received monthly - approximately 73,000.
4) All keyed data will be returned via standard electronic transmission, such as FTP or other agreed upon transmission method or tile may be attached as a text document to an e-mail.
5) The term of the contract shall be for a period of twelve (12) months. The contract shall begin January 1, 2011 and end December 31, 2011.
[B] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
[C] Eligibility: Onshore (USA Only)
[D] Budget: Looking for Proposals
