[A] Scope of Service:
1) Contractor will provide work plan, including sample design, schedule, quality control process, and interview guidelines.
2) Contractor will prepare draft of notifications and survey. Start Data collection, beginning with mailing of pre-notification letters and ending with conclusion of phone follow-up interviews.
3) Proposer shall conduct statewide and plan-level analysis of the collected data.
4) Proposer’s survey must use a mixed mode for data collection, consisting of, at a minimum, mailing self-administered questionnaires, followed by telephone interviews with non-respondents to the mail portion.
5) There will be at least five but no more than 10 questions that will be included in the survey and analysis.
6) Administrator will provide the content of the survey questionnaire to the contractor in a draft final format.
7) The contract award from this RFP will be for a period beginning approximately December 1, 2010 through December 31, 2011.
[B] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
