[A] Scope of Service:
1) Contract dates: July 1, 2011 through June 30, 2012 with the option for 4 one-year renewals.
2) Medical Transcription Services of up to 25,000 lines per month (300,000 lines per year) (Estimate)
3) Provide verbatim transcription
4) Ensure an error rate not to exceed 2 errors per page.
5) Transcribe and return all dictation within 24 hours (7 days a week, 365 days a year) except for “stat” work which should be returned with 2 hours.
6) Vendor must provide the capability to use hand-held dictation equipment to dictate and transfer voice files via Internet protocol to a secured web site with transcribed dictation returned via secure website.
[B] Work Performance:
Performance of the work will be offsite; Contractor needs to carry work on their office premises.
