1) Contractor needs to conduct telephone survey, with immunization provider record check (PRC) verification, established in response to the immediate and continuing need for quality and timely data pertaining to the immunization coverage of pre-school children across the United States.
2) Contractor needs to submit capability statement, proposal or quotation via email.
