[A] Scope of Service:
1) Contractor will design and run surveys via internet and telephone 24 hours a day, seven days a week, during the survey period.
2) Automatically gather, store, and provide data files in a consistent format that makes uploading to the primary operational database as simple as possible. Data must be provided in a format that is compatible with Excel and SPSS.
3) Assign individual pass-codes and/or user ids to approximately 17000 potential respondents.
4) Provide tailored live results on a password-protected website.
5) Provide tailored static reports.
6) Provide raw data in Excel format
7) Ensure confidentiality of all data.
8) Ensure verification of respondent prior to accessing the survey.
9) E-mail participants to prompt navigation to the survey via web site and provide the phone number for accessing the IVR survey.
10) Provide call center support for respondents.
11) Contract that will begin July 1, 2011 and end June 30, 2012.

[B] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
