[A] Scope of Service:
1) The survey will be conducted for up to thirty-six (36) months, beginning June 2011 through May, 2014.
2) Two hundred (200) telephone interviews will be conducted per month in English and Spanish, for a total of seven thousand two hundred (7,200) completed interviews.
3) We will provide the questionnaire which is currently used for the survey.
4) We require that data collection occur using a Computer Assisted Telephone Interviewing (CATI) system.
5) Interviews will be conducted weekday evenings and weekend days and evenings.
6) Two hundred (200) interviews will be conducted by the last day of the month, each month of the contract duration.
7) A minimum of 2.5% of the interviews will be conducted in Spanish each month
8) Contractor will provide monthly report of the interviews conducted.

[B] Work Performance:
Performance of the work will be offsite; Contractor needs to carry work on their office premises within USA.
