[A] Scope of Service:
1) The bidders awarded contracts will provide statewide data entry services on an as needed basis for any State agency. Agencies may require services both on-site and off-site.
2) The contract resulting from this RFB will be for the period July 1, 2011 through June 30, 2012,
3) Vendor needs to pick-up required documents from our office premises for Offsite Data Entry Services.
4) Since some Authorized Users require on-site data entry services, we intends to award this contract to at least one (1) contractor who can meet the requirement of providing up to seven (7) people, including data entry staff, clerical pre-editing staff, and a supervisor, approximately two (2) or more days per month.
5) Contractor shall maintain a 99.5% error free rate on work that is keyed without verification
6) Contractor shall maintain an average on-time delivery rate of 95% per month.

Performance of the work will be Offsite and Onsite.
