[A] Scope of Service:
1) Data entry services of forms. Forms may be single page or multi-page forms.
2) Forms consist of an average monthly total of 6,000 forms.
3) Receive forms from staff weekly. The forms will be mailed or delivered to the contractor
4) Scan forms with Ascent Capture or other scanning system with the resulting image file linked to appropriate data records. Data is extracted from forms.
5) Inspect images for clarity and re-scan if necessary.
6) Forms are to be entered via a blind double key entry and data is to be broken up into five (5) different record types. Header information, detail information, shipping information, footer information, and image file information.
7) Data is to be verified in order to ensure that all processed documents are error free.
8) Output file of data and images are to be compressed and loaded onto an FTP server.
9) Approximate volume of the work will be 1.2 million keystrokes per month.
10) Contract period will be for one year starting July 1, 2011.
